• AMB-FUBINACA is among the most potent synthetic cannabinoids known (CB1 K i ≈0.9 nM). A few milligrams can dose hundreds of people; weighing with a scale no better than 1 mg is unsafe—volumetric or pre-dilution onto inert herb is essential. • Responsible for >50 fatalities in New Zealand (2017–2019) and the 2016 “zombie” outbreak in Brooklyn, USA. Deaths often involve seizures, hyperthermia, pulmonary edema and cardiac arrest. • Many ‘spice’, ‘K2’, ‘24K Gold’ or ‘AK-47’ herbal blends have highly variable hotspots; even mixing cannot guarantee uniform concentration. • Metabolized mainly by CYP3A4; strong inhibitors (e.g., ketoconazole, grapefruit) may dramatically prolong and intensify effects. • No known antidote; treatment is supportive (benzodiazepines for agitation/seizure, IV fluids & cooling for hyperthermia).
