A few milligrams can dose hundreds of people; weighing with a scale no better than 1 mg is unsafeβvolumetric or pre-dilution onto inert herb is essential. β’ Responsible for >50 fatalities in New Zealand (2017β2019) and the 2016 βzombieβ outbreak in Brooklyn, USA. Deaths often involve seizures, hyperthermia, pulmonary edema and cardiac arrest. β’ Many βspiceβ, βK2β, β24K Goldβ or βAK-47β herbal blends have highly variable hotspots; even mixing cannot guarantee uniform concentration. β’ Metabolized mainly by CYP3A4; strong inhibitors (e.g., ketoconazole, grapefruit) may dramatically prolong and intensify effects. β’ No known antidote; treatment is supportive (benzodiazepines for agitation/seizure, IV fluids & cooling for hyperthermia).
